Admission Packet

Upon admission you should receive a packet with Education Materials enclosed. The information you receive is meant to help make your stay more comfortable and help you understand your care a little more. As you recieve more information regarding your care, please ask your nurse to place that information in your Admission Packet. Please be sure you take your Packet home with you when you are discharged. Below are links to some of the information that will be included in your Packet:

As a hospital, we are dedicated to protecting your health information and your privacy. At admission/registration, you will be given a copy of our "Notice of Privacy Practices". This notice describes how health information about you may be used and how you can get access to this information.
